Vocational Occupational Therapist Fit Note Pilot

We know that helping people return to work is an important part of the UK Government’s public health agenda, especially as more people are being signed off sick for longer periods. Good work is often beneficial for health and wellbeing, while being out of work—particularly for a long time—can have a negative impact.

At the moment, GPs are finding it difficult to have in‑depth work‑focused conversations because of limited time and capacity. That’s why we’re running a three‑month pilot where Occupational Therapists take a leading role in vocational rehabilitation within primary care.

Occupational Therapists are trained in both physical and mental health and focus on how people manage their daily activities, including work. They apply a holistic approach, which means that they look at the whole picture and understand the impact of health on someone’s ability to work. They are able to issue fit notes and provide Allied Health Work Reports to recommend reasonable adjustments and personalised support

During this pilot, patients can be referred to an Occupational Therapist who will be able to discuss ways to help return to work following an initial FIT note being issued by the GP.  These appointments will be by phone or online. Face to face appointments may be able to be accommodated if required.

The pilot will operate from 15th May – 24th July 2026.
