Our Clinics and Services

Asthma

The first part of the annual review process will ask you to answer some questions to provide some information in relation to your asthma symptoms. Invites will be sent either via SMS, Email or Post and will include an asthma questionnaire.

The information you provide relating to your asthma symptoms will then be reviewed alongside your medical record by a clinician. Based on this review there may be follow up with you via a telephone or video consultation with an asthma nurse, and we will contact you to arrange this.

If no follow up is required you will be sent an individualised care plan including information to help you manage your asthma, links to useful self-care websites and what to do if your symptoms worsen.

Child Health

All the Doctors and Nurses in our Surgery are experienced at looking after children.

When your baby is born our Health Visitors will visit you at home for the first few weeks. After this you can attend baby clinic or request a home visit if you are unable to come into the Surgery.

At 6 to 8 weeks a baby check will be arranged with your GP along with initial immunisations.

Chiropody (Podiatry)

The Chiropody Service is available to patients with special needs such as impaired circulation or disabling foot conditions and also elderly patients with a clinical chiropodial need. All patients will, upon referral, be provided with an assessment leading to treatment, advice or referral to a hospital specialist service. A community service is available for those patients unable to travel to the Surgery.

Chronic Obstructive Pulmonary Disease (COPD)

The first part of the annual review process we will ask you to answer some questions to provide some information in relation to your COPD symptoms.

You will be sent an invite either by SMS, Email or Post which will include a COPD questionnaire.

Depending upon your answers and your other medical conditions, the next part is for all the information you have provided relating to your COPD symptoms to be reviewed, alongside your medical record, by a clinician. Based on this review there may be follow up with you via a telephone or video consultation with a respiratory nurse and we will contact you to arrange this.

If no follow up is required you will be sent an individualised care plan including information to help you manage your COPD, with inks to useful self-care websites and what to do if your symptoms worsen.

Coronary Heart Disease

All patients diagnosed with CHD are invited for a yearly review.

The first part is an appointment with one of our Health Care Assistants (HCA), who will take a blood test, and also check BP, height and weight. We will then contact you with your results, and if needed book further follow up with a CHD trained Nurse who will discuss your overall care.

Diabetes

The first part of the annual review process will ask you to attend an appointment with one of our Health Care Assistants for your Diabetes annual review assessment. This will include your blood tests, foot check, height and weight.

Once your blood test results are back they will be reviewed. Any follow up required will be identified and is likely to be one of the following:

  • Follow up with the diabetes nurse - We will contact you to arrange a telephone/video or face to face appointment with the diabetes nurse
  • Follow up by your GP - We will contact you to arrange a telephone/video review with your GP.
  • No specific follow up required – We will send you your results in a diabetes care plan which includes useful information and the date when your next blood test is due.

Please note it may take up to 3 weeks for your results to be received and reviewed.

Hypertension

Hypertension (High Blood Pressure BP) does not cause symptoms, but if it is not treated it can cause damage to the heart, kidneys and brain. Hypertension increases your risk of heart attack and stroke. Good control can significantly reduce your risk of developing these serious illnesses.

Patients with hypertension are invited for an annual review consisting of BP readings and a blood test, usually in the month of your birthday.

Prior to your appointment, we will ask you to complete a weeks’ worth of home BP readings and bring this along to your blood test appointment.

Please contact the surgery to book your blood test when your annual review is due.

Your blood pressure readings will be reviewed, and you will be contacted if any further follow up is needed.

Health Care Assistants General Clinics

Our Health Care Assistants run regular clinics for the following:

  • Blood Tests
  • ECGs
  • Weight Checks
  • BP checks
  • Vaccinations (not child immunisations)

Maternity Care

If you think you are pregnant please call in to the surgery and inform reception. You will then be given a 'Bounty Pack' which includes how to self refer to the midwifery team and other important information.

You can also self refer online.

Your antenatal care is shared between you and your GP if you have a low risk pregnancy. If you have a high risk pregnancy you will be referred to an Obstetrician.

You will be offered screening during your pregnancy including screening for infectious diseases and Down’s Syndrome.

Our Health Visitors are informed once the pregnancy is confirmed and will be in contact with you during the pregnancy and after you give birth.

Travel Vaccinations

If you are thinking of travelling outside the UK, start preparing for your trip, especially long trips, at least 1 month before you go.

You don't always need vaccinations to travel abroad. If you do, the type of vaccination depends on which country you're visiting and what you're doing. Please be aware that some of the vaccinations require more than one dose and some do not become effective until several weeks after the injection.

Royal Crescent & Preston Road Patients

Please contact reception to book your initial Travel Telephone Appointment where your risk will be assessed, and plans made for any vaccinations if needed.

Dorcester Road Patients

If you require any vaccinations relating to foreign travel, please complete our online travel form.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the Practice Nurse to receive the vaccinations. These vaccines might need to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription, and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS. We do not provide prescriptions for antimalarials which can be obtained without a prescription from most pharmacies.

Your rights & EU rules

If you are travelling to Europe the EU has published useful information for travellers on the European website.

As an EU national, you enjoy the right of free movement. This means you’re entitled to travel, work and live in another EU country. If you’re a citizen of a Schengen country – which is most EU countries – you’re also free to travel to other Schengen countries without the need for border checks.

As an EU national, you can also benefit from EU-wide passenger rights for travel into, out of or within the EU by air, rail, bus/coach or ship. You can travel with pets and other animals in the EU provided you comply with the rules (pet passports). There are EU limits on taking alcohol, tobacco or cash with you to another EU country. Each EU country can set its own limits or restrictions, so check before you go.
